Dallas Cosmos Soccer Club (DCSC) is making tournament Check-In EASY…featuring our “Team E- CHECK-IN®” process. By making the process more convenient, we hope you and your team will have a much more enjoyable tournament experience.
Use our EASY “Check List” to make sure you have all of your required documents. The “Check List” doubles as ourTournament Contact Information Sheet & your Document Check List. It’s available in both PDF & Word formats.
While we have simplified the pre-tournament experience, all tournament teams and participants are expected to abide by all game day requests & requirements…in particular as they relate to Medical Releases, Birth Certificates, Registration Forms and Rosters.

Team E- CHECK-IN® Process:
Step (1) Open / Download Tournament “Check List“ (PDF/ Word Doc) - try our Word Doc version…it’s interactive (form fill)
Step (2) Complete “Contact Information Sheet” (top box) – this form is a required document
Step (3) Identify appropriate section for your team (i.e., Competitive is section 1) on the “Check List” and gather all required documents (*see DETAILS below)
Step (4) Attach (scan) ALL required documents to email with “Subject line”: Age Group / Gender / Club / Team Name
– Example Email Subject Line: U13 Girls Cosmos 03G Black - Include contact info in the body of the email
Step (5) Send above email to: register@dallascosmos.com
Step (6) BE SURE to create your “Event Roster” in GotSoccer to include ALL players for the particular tournament (“Event”)

DETAILS: Competitive U11-U19 - required for check-in approval -
Accepted Rosters: Official Team Roster from either your State Association or US Club Soccer – must be properly executed by your State association w/ signatures and dated for the current soccer year.

US Club Soccer Roster – handwritten roster will not be accepted

Non-NTSSA Rosters must include USYS Travel Authorization w/ signature of approval by State Assn.
Accepted Guest Player Release(s): Required for any player NOT listed on your official team roster – max. 5 per team.

NTSSA Guest Player Release form with Player Registration # and all appropriate signatures

US Club Soccer Roster accepted with properly executed Player Loan Form(s)

Traveling Teams w/ Non-NTSSA Rosters must provide a Completed State Ass. Guest Player Release or Approved State Assn. Roster listing guest player(s)
**Guest player(s) must be added to GotSoccer roster prior to submitting paperwork for Team E- CHECK-IN**
Medical Release Agreement: By signing the Medical Release Agreement, the manager and coach agree to have at each game a Medical Release Form (completed, including parent/guardian signature) for each player listed on the Game Roster.
**During the tournament, Tournament Officials may / can approach your Team Representative (coach / manager) and request proof of Medical Releases. Teams who do not comply may be forced to forfeit current and/or previous matches and may be removed from the tournament**

Accepted Rosters: Official Team Roster from either your State Association or US Club Soccer – must be properly executed by your State association w/ signatures and dated for the current soccer year.
